Methylprednisolone is a synthetic glucocorticoid with potent anti-inflammatory and immunosuppressant properties, widely used in the management of a broad range of inflammatory, allergic, autoimmune, and haematological disorders.
Methylprednisolone exerts its therapeutic effects by binding to intracellular glucocorticoid receptors in the cytoplasm. Upon binding, the receptor-ligand complex translocates to the cell nucleus, where it regulates the transcription of multiple genes involved in the inflammatory cascade. This genomic action leads to the suppression of pro-inflammatory cytokines (including interleukins IL-1, IL-2, IL-6, and TNF-α), inhibition of phospholipase A2 (reducing arachidonic acid release and subsequent prostaglandin and leukotriene synthesis), downregulation of adhesion molecules, and stabilization of lysosomal membranes. The result is a broad, potent reduction in inflammation, oedema, and immune-mediated tissue damage.
Compared to prednisolone, methylprednisolone has less mineralocorticoid activity (reduced sodium and water retention), making it preferable in patients at risk of fluid overload, hypertension, or heart failure. It also penetrates the central nervous system more effectively, making it useful in certain neurological conditions.
